For most healthcare institutions and hospitals, patient charts containing the medical history of their patients are kept in order for assigned physicians to have a basis for making their medical diagnoses. Current medical complaints and conditions need to be traced prior to prescribing and advising new medicines and health examinations such as laboratory examinations and cardiology exercises. One wrong advice could lead to patient liability and may worsen the situation of the patient.
